All suspected cases and close contacts of a suspected or confirmed COVID-19 case identified (isolated & sent home) at ECCE, Primary and Secondary schools, among students and staff members, should be captured. This digital initiative has been implemented to aid in the capturing of that data.
Principals are tasked with the responsibility of submitting their data via the two (2) forms below:
1. DAILY COUNT OF SUSPECTED CASES OR CONTACTS FORM
2. REPORTING OF SUSPECTED COVID-19 CASES FORM
